For Food and Clothing:

A)    Photo ID for person applying for assistance

B)     Current proof of address showing the name of person applying.

For example:

1. Current lease

2. Current bill

3. Government aid paperwork

4. Child's report card

(If none of these documents are available, you may complete a residency affidavit form available at the Co-op office)

C)    ID's for each family member in household.

For Example:

1. Social Security card

2. Birth certificate

3. School ID

For financial assistance

 A), B), and C) above

And, a current lease contract in your name and a utility bill if applicable.
